Competition Entry Procedure

1. Be sure to fill in both the top and the bottom form. One form is for our records and the other will be your copy. Make a card out for each category and size, i.e., small color or small black and white

2. Fill in your name and date on first line

3. Fill in your membership ID on Second line.

4. Check Prints or color and either Small or Large.

5. If the print is a male-up, select Make-up also and fill in the dates that make-up representing.

6. Fill in each print Title or DPI Print on each respective number print number line.
